Click on the appropriate link below to get the Application Agreement page for the league you are applying.
Read the information and click on either the { I AGREE } or { I DISAGREE} button and follow the directions.
When you have completed your application click submit. You will be directed to a payment page. If you are paying by check make sure the pay by check circle is highlighted and click submit. If you are paying by credit card you will need to complete the required information.
Applications must be completed ONLINE by DECEMBER 15, 2009 for SCSCL and PMSL teams.
